Causes of sciatica

Sciatica pain is caused by irritation and inflammation of the sciatic nerve, a large nerve that branches off the spinal cord. The sciatic nerve is the largest nerve in the body, about the diameter of a finger. Their nerve fibers begin on 4 and 5 lumbar vertebrae (L4, L5) and first sacral segments. It runs vertically on the back of the thigh behind the knee, branching into the calf muscle (hamstring) and lower heel. The pain usually affects one side of the body from the buttocks and slowly radiates to the back or side of the thigh. It may radiate to the leg below of the knee and may even reach the heel. It is described as:
• Burning
• Sharp
• Dull pain
• tingling or numbness
• continuous or intermittent pain

These painful symptoms can progress to severe pain in the heel or foot, with numbness, tingling and weakness progressive in the affected limb. Other symptoms are:
• The pain is worse when walking
• The pain worsens with prolonged sitting
• Pain may get worse when trying to stand from a sitting down position
• The pain gets worse when bending backwards, coughing, sneezing or straining

There are different types of spinal disorders can cause spinal nerve compression, resulting in pain and inflammation of sciatica. Causes common are:

• Spondylolisthesis

The most important step in the treatment of sciatica is to determine what is causing compression of the sciatic nerve roots. Because there are many causes, symptoms and treatment vary from patient to patient. Diagnostic tests and examinations must be complete and exhaustive. Imaging methods as neurography MR has been effective in diagnosing 95% of patients with severe sciatica.
